Each area of your life is discussed. When there are areas of concern or need, specific resources and services within your community are recommended. This care plan is most beneficial when reviewed together after completion. Priorities can be set and the very next step planned. Having this written plan brings some sense of calm and order when there is much to do or you are feeling overwhelmed.
For those on a tighter budget or wanting minimal professional help, this service of creating a care plan is perhaps the best option. You are given all the resources you need to put the services in place and monitor them yourself. Also, once we have this information, should you ever need support during a crisis, we have the information on hand to jump into action.
