Comprehensive Assessment

withClientsThis is usually the best way for us to assess how we can help. Our geriatric care managers meet with the client and whenever appropriate, their family members. The assessment starts with the first phone call. During the assessment interview you can expect our care manager to ask a lot of questions. We want to get the full picture of your circumstances, options, hopes and goals. To do so we will briefly review all important elements of your life and quickly focus on the areas of most concern. If desired, we can then create a comprehensive care plan. This care plan is your road map, full of resources and recommendations for the next steps. This assessment includes a review of the following areas: Medical Concerns , Social, Home Environment & Physical Safety, Legal Needs and if desired, Financial Overview.
